Records Requests
Patients and authorized individuals can request copies of medical records from Hand & Orthopedic Physical Therapy Specialists by following the link below. Records requests are handled securely and in accordance with applicable privacy requirements to protect personal health information.
Standard medical record requests are typically completed within 2–3 business days. FMLA and disability form requests generally take approximately 7 business days due to additional information requirements and physician review.
