Know Your Insurance Benefits
Because you are ultimately responsible for your account regardless of insurance coverage, we highly recommend that you contact your insurance provider directly to learn your physical therapy benefits. Our office will provide you with an insurance benefit worksheet that will list your physical/occupational therapy benefits as they were quoted to us. Remember, benefits quoted are not a guarantee of payment.

What is Direct Access to Physical Therapy?
Utah law supports the patients’ right to choose their own physical therapist, regardless of physician recommendations.
Please consider us Your Physical Therapy Clinic. As a comprehensive therapy company, we cover the full spectrum of rehabilitation services. You can be assured that you will receive the same expertise with every condition we treat. We are happy to offer convenient options for your physical therapy program with our four clinic locations and extended hours of service.
Please check with your insurance company prior to your visit to learn if your plan requires a physician referral for physical or occupational therapy.
What to Expect During your First Visit
It takes roughly 10-15 minutes to process a new patient’s paperwork. A patient advocate will meet with you to review your estimated health insurance benefits. This is why we ask you to come early. Please bring insurance card(s), photo ID and prescription from your referring doctor (if applicable).
