New Patients

Welcome to Hand & Orthopedic Rehabilitation Specialists. We are excited to get you back to your daily activities.

Please follow these steps to schedule your first appointment:

 

  1. Choose a location from the clinic locations tab that best suits your needs (please note that not all specialties are offered at each location).
  2. Call the receptionist to schedule an appointment. Please have your schedule, referring physician and insurance information ready.
  3. Print one each of the following forms or go to the forms section: Patient Intake Form, Financial Policy, HIPPA Notification/Agreement. (You may still be required to fill out additional forms at your appointment but having these form filled out prior to your appointment will lessen your waiting time).
  4. If you have access to a fax machine you may fax completed forms to the fax number of your clinic location. Please bring your completed forms with you to your first visit even if you have faxed them.

Return Patients

Tips:

  • Because we want you to get the appointment time that best suits your schedule we recommend that you schedule all follow-up appointments at least a week in advance.
  • Please have your calendar available to consult when you’re scheduling.
  • Always remember to get a print out from the front desk of all your scheduled appointments.
  • We offer E-mail appointment reminders. If you think this might help you please sign up at our front desk.

Reminder: Our patient’s physical therapy treatments are of utmost importance to us. They are the key to rehabilitating our patients. If you don’t come then we can’t get you better. Also, if you cancel your appointment without enough notice then we are unable to help another patient in need during that time. To discourage valuable time wasted by patient’s failure to show we have a 24 hour cancellation policy. If you do not notify our office that you cannot make your appointment within 24 hours of your scheduled appointment time you will be charged $45.

What to expect or preparing for your first visit

It takes roughly 15-20 minutes to process a new patient’s paperwork. We also like to meet with each of our new patients and go over their estimated health insurance benefits. This is why we ask you to come early. Please bring insurance card(s), photo id and prescription from your referring doctor (if applicable).

Please dress comfortably in clothes that are suitable to move around in. If you have an extremity injury please wear clothing that is loose enough or allows access to the injured area.

Expected length of treatment

An initial evaluation is preformed on your first visit and that appointment can last up to two hours.

What can I expect after treatment

The results of your initial treatment can be varied. Some people will have immediate relief, while others may have increased pain. Both are normal responses to treatment. If you experience immediate relief is it important to continue therapy for long term benefit. If you experience increased pain, this is not necessarily a bad thing. The treatment and retraining of muscles can cause initial pain and inflammation, but with continued therapy you will experience relief.
